TMV Service Record Sheet

A record for in-service testing of thermostatic mixing valves, covering temperature, fail-safe testing and cleaning.

How to use this sheet

  1. Give each valve an asset number and record its location and type (TMV2 or TMV3)
  2. Measure the mixed water temperature and compare it with the set point
  3. Carry out a fail-safe test by isolating the cold supply
  4. Record whether strainers and check valves were cleaned
  5. Write the next due date so nothing is missed

Keep completed records for at least five years. General guidance only: follow your own risk assessment where it sets different checks or frequencies.

Frequently asked questions

How often should TMVs be serviced?

At least annually in most settings, with frequencies in healthcare set by performance results under HTM 04-01.

Is a fail-safe test always needed?

Yes. It confirms the valve shuts off hot water if the cold supply fails, which protects against scalding.
